When Is a Spinal X-Ray Ordered for Back or Neck Pain?

A spinal X-ray is a practical and commonly used first-line investigation when your GP needs information about the bony structure of the spine. It is fast, low-dose, and provides useful information to guide initial management in a wide range of presentations.

Common reasons for a spinal X-ray referral include:
– New or persistent back or neck pain — particularly when a spine related cause is possible
– Pain following a fall, minor trauma, or sudden onset
– Monitoring of known spinal conditions such as scoliosis or ankylosing spondylitis
– Assessment of vertebral alignment and curvature
– Routine pre-physiotherapy or pre-specialist assessment
– Screening for age-related bony changes in older adults
– Assessment of transitional anatomy contributing to symptoms

While a spinal X-ray does not directly visualise the discs, nerve roots, or spinal cord (MRI is the appropriate investigation for these), it provides your GP with a clear starting point for investigating spinal symptoms. Walk-in spinal X-rays are available at Epoch Medical Imaging in St Ives Shopping Village.

What Can a Lumbar Spine X-Ray Detect?

A lumbar spine X-ray provides a clear image of the five lumbar vertebrae and the sacrum — giving your doctor useful information about the bony architecture of the lower back.

What a lumbar spine X-ray can detect:
– Vertebral alignment and any slippage (spondylolisthesis)
– Disc space narrowing — an indirect indicator of disc degeneration
– Bone spur (osteophyte) formation at the vertebral end-plates
– Vertebral fractures — including compression fractures in osteoporosis
– Scoliosis — curvature of the spine, and its degree
– Arthritic changes in the facet joints and disc spaces
– Transitional vertebral anatomy (such as a sacralized L5)
– Structural changes following previous spinal surgery

Spinal X-Ray vs MRI: Which Investigation Is Right for You?

A spinal X-ray and an MRI provide different types of information about the spine — and understanding the difference helps explain why your doctor may recommend one, the other, or both at different stages of your assessment.

A spinal X-ray is the appropriate first investigation for most back pain presentations — particularly when bony assessment is the primary clinical goal. MRI is recommended when nerve involvement is suspected (radiating pain, numbness, weakness) or when symptoms persist despite treatment. Both are available at Epoch Medical Imaging located in St Ives Shopping Village.

Cervical, Thoracic & Lumbar Spine: Understanding the Regions

The spine is divided into three main regions, and spinal X-rays are typically performed for one region at a time based on where your symptoms are located.

Your GP will specify which region to X-ray based on where your symptoms are located. In some cases, more than one region may be requested. Walk-in spinal X-rays are available at Epoch Medical Imaging located in St Ives Shopping Village during all standard opening hours — simply bring your [GP referral] and our team will assist you promptly.
