Image-Guided Biopsy — Ultrasound & CT Guided Tissue Sampling

Image-guided biopsies at Epoch Medical Imaging, St Ives. Ultrasound and CT-guided tissue sampling with specialist reporting. Serving the North Shore.

What Is an Image-Guided Biopsy and Why Is It More Accurate?

An image-guided biopsy is a minimally invasive procedure in which a small sample of tissue is taken from a lump, lesion, or organ using a needle — with real-time imaging used to guide the needle precisely to the target. It is a far more accurate and safer approach than an unguided (blind) biopsy, and for most indications it provides all the tissue information needed for a definitive diagnosis without requiring surgery.

Why image guidance makes the difference:
– The radiologist can see the needle tip at all times during the procedure — confirming it is within the target lesion or organ before any tissue is sampled.
– Surrounding structures such as blood vessels, bowel, and nerves are clearly visualised, allowing them to be avoided.
– The most representative part of a lesion — avoiding necrotic (dead) central areas — can be specifically targeted, improving diagnostic yield.
– The procedure can be completed with confidence in a single appointment, reducing the need for repeat biopsies.

At Epoch Medical Imaging in St Ives, image-guided biopsies are performed by specialist radiologists using either ultrasound or CT guidance, depending on the target location and anatomy.

Ultrasound vs CT-Guided Biopsy: When Is Each Used?

The choice between ultrasound and CT guidance for a biopsy depends on the location of the target, its depth, and its relationship to surrounding structures.

Ultrasound is preferred for most superficial and many abdominal targets — it provides continuous real-time needle visualisation without radiation. CT is used for deep targets, lung lesions, bony lesions, and situations where bowel or other structures need to be carefully mapped before proceeding.

At Epoch Medical Imaging in St Ives, our specialist radiologists select the most appropriate modality for each biopsy to maximise accuracy and safety.

Core Biopsy vs Fine Needle Aspiration (FNA): What Is the Difference?

Two main biopsy techniques are used at Epoch Medical Imaging — the choice depends on the target organ, the likely diagnosis, and what information is needed from the sample.

Fine Needle Aspiration (FNA):
– A very fine needle (typically 22–25 gauge) is used to aspirate cells from the target.
– Provides cells for cytological analysis — a cellular sample assessed by the pathologist.
– Used most commonly for thyroid nodules, lymph nodes, and salivary gland lesions.
– Very low procedural risk; minimal discomfort; often performed without local anaesthetic or with topical anaesthetic only.

Core Needle Biopsy:
– A larger needle (typically 16–18 gauge) with a spring-loaded cutting mechanism takes a small cylinder of intact tissue.
– Provides a tissue core for histological analysis — preserving tissue architecture for more detailed assessment.
– Used for breast, liver, kidney, lung, bone, and soft tissue lesions where a tissue diagnosis is required.
– Performed under local anaesthetic; minor pressure sensation during the sample.
– Provides more diagnostic information than FNA for most solid lesion types.

Your referring doctor or our radiologist will specify which technique is most appropriate for your biopsy.

How Long Does a Biopsy Take and When Are Results Available?

An image-guided biopsy at Epoch Medical Imaging is a well-organised, time-efficient procedure — most patients are surprised by how manageable the experience is.

Duration on the day:
– Total appointment: Allow 30 to 90 minutes, including preparation, the procedure, and a short observation period.
– The actual sampling component takes only a few minutes once imaging has been performed and the needle is in position.
– Most patients can go home within 30 to 60 minutes of completing the procedure.

When results are available:
– Pathology results are not available on the day of your biopsy — samples are sent to a specialist pathology laboratory for analysis.
– FNA cytology results: Typically 2 to 5 business days.
– Core biopsy histology results: Typically 3 to 7 business days, depending on whether special stains or additional tests are required.
– Your GP or referring specialist will receive the results and contact you to discuss findings and next steps.
– If your results are urgent (for example, before a planned surgical procedure), please inform our team when booking so we can request expedited processing.
