Diagnostic mammograms at Epoch Medical Imaging, St Ives. Expert breast imaging for symptoms, lumps and follow-up assessment. Serving the North Shore.
What Is 3D Mammography (Tomosynthesis) and How Is It Different?
3D mammography — also called digital breast tomosynthesis — is an advanced form of mammography that produces multiple thin, layered images of the breast tissue in addition to the standard 2D views. Instead of a single flat image, the machine takes a series of images from different angles and reconstructs them into a three-dimensional representation of the breast that the radiologist can scroll through slice by slice.
How it differs from standard 2D mammography:
– Standard 2D: A single flat image of each breast from two standard angles.
– 3D tomosynthesis: Multiple thin image slices through the breast tissue, allowing tissue to be examined layer by layer.
The key advantage of 3D is that it dramatically reduces the overlapping effect of breast tissue — a major limitation of standard 2D mammography where normal tissue can overlap and obscure findings. At Epoch Medical Imaging in St Ives, advanced mammography technology provides our patients across the North Shore with the highest quality breast imaging available.
Does 3D Mammography Detect More Cancers Than Standard 2D?
Clinical evidence strongly supports several meaningful advantages of 3D mammography over standard 2D mammography — particularly for women with dense breast tissue.
Key findings from research evidence:
– Improved detection: Studies show that 3D mammography detects more invasive cancers per 1,000 women screened compared to 2D alone.
– Fewer false positives: Because tissue overlapping is significantly reduced, there are fewer instances where normal overlapping tissue is mistaken for an abnormality — resulting in fewer unnecessary recall appointments for additional imaging.
– Better characterisation: The layered imaging allows radiologists to more confidently assess whether a finding represents a true lesion or normal overlapping tissue.
The improvement in detection combined with the reduction in false positive recalls represents a meaningful clinical benefit — particularly for women who have previously experienced the anxiety of being called back after a 2D mammogram for a finding that turned out to be normal overlapping tissue.
Is 3D Mammography Better for Dense Breast Tissue?
Yes — women with dense breast tissue benefit most significantly from 3D mammography, and this is where the advantage over standard 2D is most pronounced.
Why dense breast tissue matters for mammography:
– Dense breast tissue appears white on a mammogram — and certain findings also appear white. In a 2D mammogram, these can overlap and make it harder to identify small lesions.
– Dense breasts are associated with a modestly higher personal risk of breast cancer.
– 3D tomosynthesis allows the radiologist to scroll through the breast in thin slices — significantly reducing the masking effect of dense tissue and improving the ability to see through it clearly.
Whether you have dense breast tissue is determined by the radiologist when reporting your mammogram, and is noted in your report. 3D Mammography Radiation Dose: Is It Safe?
Yes — 3D mammography is considered safe, and the radiation dose remains within established guidelines for breast screening.
Radiation dose considerations:
– 3D tomosynthesis involves a slightly higher radiation dose than standard 2D mammography taken in isolation.
– The radiation dose from a mammogram — 2D or 3D — is very low, equivalent to a few weeks of natural background radiation from the environment.
– The benefit of improved cancer detection and fewer false positive recalls significantly outweighs the minimal radiation exposure for women in the recommended screening age groups.
At Epoch Medical Imaging located in St Ives, we use modern mammography equipment designed to deliver the best possible image quality at the lowest practical radiation dose — in line with national guidelines and best practice.
